Making An Appointment

APPOINTMENT BOOKING TIMES:
MONDAY TO FRIDAY 
8:30 am to 11:30 am  and  1pm to 3:30 pm

To make an appointment you need to do 2 things: 1) REGISTER and 2) EMAIL the office for an actual appointment date.

  1. REGISTER: You can register by clicking here. Please carefully and accurately fill out the registration form.  After you register, the system will provide you with a registration number which you BRING to the medical appointment.
  2. MEDICAL APPOINTMENT:   To make an appointment, please EMAIL us at doctorlyndon@LMAS.ca with:
  • The Names and Registration number of each individual requiring medicals
  • Telephone contact numbers
  • Country (Canadian, US, Australian ) of immigration required
  • Your future/or previous interview date with the US consulate in Montreal - you need a scheduled interview date before you can have a medical exam.
  • Optional dates and times available for appointments - We need up to 10 business days to prepare your report.

We will contact you shortly and can accommodate quick appointments. No fasting is required for appointment.

For Applicants who have received a letter from the U.S. BORDER SERVICES or have a DUI CHARGE. The fee for this assessment is $ 500, please contact our office via email and we will advise you as to the next steps.

What to Bring for Your Appointment 

    • 1 US sized Passport Photo (For Each Applicant)
    • Please bring your registration number that you received when you booked the appointment with my office
    • The complete intended address in the US with zip code.
    • Letter with your Name, Case Number & Interview Date
    • Your Passport
    • If you are asked to repeat the medical because it has expired, then the entire process has to be repeated with payment of the same fee.
    • Completed Vaccine Checklist for each Applicant
    • Payment in Cash and Debit or Credit Card Only:

All Fees are in Canadian Dollars and includes Taxes, X-Ray & Lab.

  • VACCINATIONS ARE MANDATORY AND ARE REQUIRED BY THE US GOVERNMENT. Please bring Proof of Vacination to the appointment.
  • If you received vaccinations outside Canada please have you family doctor provide confirmation of the vaccinations before the appointment.
  • Vaccination Record: Please see Guide Below (for Your Age Group ONLY)and get Records/Vaccines from your Family Doctor or Walk In Clinic.  We do Not give Vaccines in the Office.

What To Expect After The Appointment

Please do not phone our office as you will be contacted by the office within a few days to pick up a sealed brown envelope to take for your interview at the US Consulate in Montreal.
